All physical (CIPPE) forms pages 1 thru 6 need to be completed (page 6 completed by a physician) and handed in to your coach or Ms. Rickert before participating in the first official practice.
The additional Forms A-D will be handed out to coaches when they are updated. The forms on the website are not updated.
