IMMUNIZATIONS

The following immunizations are required prior to entering the listed grade level (before the start of the school year).

All Grades

• 4 doses of tetanus, diphtheria and acellular pertussis* (1 dose on or after the 4th birthday)

• 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose

given) **

• 2 doses of measles, mumps, rubella ***

• 3 doses of Hepatitis B

• 2 doses of varicella (chickenpox) or evidence of immunity

*Usually given as DTP or DTaP or DT or T

**A 4th dose not necessary if the 3rd dose was administered at age 4 years or older and at least 6

months after the previous dose

***Usually given as MMR

7th Grade

• 1 dose of tetanus, diphtheria, acellular pertussis (TDAP) on the first day of 7th Grade

• 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 7th Grade

12th Grade

• 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 12th Grade (If one dose

was given at 16 years of age or older, that shall count as the 12th Grade dose).

The new school immunization law now allows only a 5 day provisional period beginning the first day of the new school year.  

PHYSICAL EXAMS

Any student entering Kindergarten, 6th grade or 11th grade is required to have a physical.  If you need a form, you may contact the school nurse or print the form from the Medical Forms section.

DENTAL EXAMS 

Any student entering Kindergarten, 3rd grade , or 7th grade is required to have a dental exam.  If you need a form, you may contact the school nurse or print the form from the Medical Forms section.

General Information

If your child is exhibiting any of the following signs/symptoms, please consider keeping them at home:

  • Fever of 100.0 or greater

    • child must be fever-free, without the use of medication for 24 hours before returning to school

  • Rash (unless classified as non-communicable by a doctor)

  • Vomiting and/ or diarrhea (must be symptom free for 24 hours) 

  • Excessive coughing

  • Migraine/severe headache not relieved by medication

    • This will allow for your child to heal quicker and also help to prevent the spread of germs

  • Please notify the school nurse if your child develops a communicable disease or condition for example: head lice, impetigo, pink eye, ringworm, chicken pox, etc.

  • Please keep all phone numbers, including home, work, cell, and emergency numbers, current with school office

  • Please update the school nurse throughout the year with any new health updates and/or medication changes

Vision Screenings for all grade levels K-12

Hearing Screenings for grade levels K, 1, 2, 3, 7, & 11

Height and Weight Screenings for all grade levels K-12

Scoliosis Screenings for grade levels 6 & 7

Physical Exams for grades K, 6, & 11

Dental Exams for grades K, 3, and 7
